
The idea sounds almost too simple: eat primarily oatmeal for two days, and watch your cholesterol drop by ten percent. Yet that's precisely what happened in a carefully controlled trial at the University of Bonn, with effects persisting for at least six weeks after participants returned to their normal diets.
The study, published in Nature Communications in January 2026, focused on 32 women and men with metabolic syndrome—a cluster of conditions including excess body weight, high blood pressure, elevated blood sugar, and unhealthy blood fat levels. These conditions collectively raise the risk of heart disease and type 2 diabetes. Participants followed a strict protocol: three daily meals of oatmeal boiled in water, totaling 300 grams per day, with only small amounts of fruit or vegetables allowed. Their calorie intake dropped to approximately half their normal consumption.
A control group also reduced calories but ate a different diet without oats. Both groups saw some improvements, but the oat group's results proved far more dramatic. Their LDL cholesterol—the type that accumulates in artery walls, forming dangerous plaques—fell by ten percent. They also lost an average of two kilograms and experienced slight blood pressure reductions.
"The level of particularly harmful LDL cholesterol fell by 10 percent for them—that is a substantial reduction, although not entirely comparable to the effect of modern medications," explained Professor Marie-Christine Simon, junior professor at the Institute of Nutritional and Food Science and study co-author.
Perhaps most intriguing, the benefits persisted. Follow-up examinations at two, four, and six weeks after the two-day intervention showed cholesterol levels remained stable. The brief dietary change had triggered lasting metabolic shifts.
The mechanism appears to involve the gut microbiome. Lead author Linda Klümpen and her colleagues identified that consuming large amounts of oats increased populations of specific beneficial bacteria. These microbes break down components of the oats and produce phenolic compounds, including dihydroferulic acid and ferulic acid. Previous animal studies showed ferulic acid positively influences cholesterol metabolism; this trial suggests the same occurs in humans.
The researchers also conducted a second trial where participants consumed 80 grams of oatmeal daily for six weeks while maintaining their regular Western diet. This moderate, long-term approach produced milder effects, suggesting that short, intensive oat-based interventions might prove more effective than simply adding modest amounts of oats to an otherwise unchanged diet.
Professor Simon noted that oats once played a central role in treating diabetes before modern medications became available. This research suggests dietary interventions deserve renewed attention. "A short-term oat-based diet at regular intervals could be a well-tolerated way to keep the cholesterol level within the normal range and prevent diabetes," she said.
The researchers emphasize this approach doesn't replace medication when clinically necessary, but it could offer a simple, accessible tool for managing cholesterol, particularly for people at risk of metabolic disease. The next research phase will explore how frequently the two-day intervention should be repeated for optimal long-term protection.
